The first one involves self-monitoring of thoughts and behaviours by the patient as part of a homework exercise. A diary can be used to establish the baseline of specific unhelpful thoughts, feelings and behaviours and the frequency, intensity, severity and duration of these. Assessment and treatment strategies 33 Examples of these can then be discussed in later therapy sessions. Observation of the patient by the therapist within a session may also reveal any particularly relevant behaviours, mood shifts or the expression of strong emotions and distress, which can be explored, and the ‘hot cognitions’ being triggered at that moment in time can be identified.
